What is the ideal myo-inositol to D-chiro-inositol ratio for fertility?

Research points to a 40:1 ratio of myo-inositol to D-chiro-inositol as the combination most closely mirroring what naturally occurs in healthy ovarian tissue, and clinical research has found this specific ratio most effective at supporting ovulation compared to other tested ratios. If you have tried a myo-inositol-only product without noticing a difference, the ratio, not just the presence of inositol, may be part of the story.

Not for use during pregnancy. Always consult your healthcare provider before starting any new supplement, especially while trying to conceive or breastfeeding.

Why "just take inositol" oversimplifies the science

Myo-inositol and D-chiro-inositol are both naturally occurring forms of inositol, a nutrient involved in how cells respond to insulin and how the ovary regulates hormone signaling. Many supplements on the market contain myo-inositol alone, or combine the two forms in ratios that do not reflect how they occur naturally in the body.

A clinical trial published in the European Review for Medical and Pharmacological Sciences tested seven different ratios of myo-inositol to D-chiro-inositol, ranging from D-chiro-inositol alone up to an 80:1 ratio favoring myo-inositol. Researchers found that a 40:1 ratio was the most effective for restoring ovulation among the ratios tested, and that shifting the balance further toward D-chiro-inositol actually reduced the benefit.

What the broader research shows

A 2017 meta-analysis of nine randomized controlled trials, published in Endocrine Connections, found that myo-inositol supplementation, alone or combined with D-chiro-inositol, was associated with significant improvements in fasting insulin and insulin resistance markers across the reviewed trials.

Much of this research, including the 40:1 ratio study above, was conducted in women managing ovulatory and hormonal irregularities, since these are the populations where cycle and ovulation outcomes are most closely tracked. The underlying mechanism, supporting how ovarian tissue responds to insulin signaling, is relevant more broadly to reproductive wellness, not only to that specific research population.

What "40:1" means on a supplement label

  • Look for the specific ratio listed on the supplement facts panel, not just "myo-inositol" or "inositol blend"
  • 2,000 mg myo-inositol paired with 50 mg D-chiro-inositol reflects the 40:1 ratio used in the research above
  • Products with only myo-inositol, or with a much higher proportion of D-chiro-inositol, are not using the ratio the cited research points to

Frequently asked questions

Is the 40:1 ratio only relevant for women with a diagnosed hormonal condition?

The research establishing the 40:1 ratio was conducted in women being monitored for ovulation and hormonal markers, but the underlying mechanism, how the two inositol forms work together to support ovarian insulin signaling, is relevant to reproductive wellness more broadly. Talk with your provider about whether it fits your specific situation.

Can I take myo-inositol without D-chiro-inositol?

You can, and myo-inositol alone has research support of its own. The 40:1 ratio research specifically found the combination outperformed myo-inositol paired in less favorable ratios with D-chiro-inositol.

How long does it take to notice a difference in cycle regularity?

Clinical trials on inositol ratios typically ran for three months. Cycle-related changes are generally not something to expect within days or a single cycle.

Is Myo Chiro 40:1 safe during pregnancy?

No. Myo Chiro 40:1 is not for use during pregnancy. If you conceive while taking it, stop and talk with your healthcare provider.

Can I take Myo Chiro 40:1 while breastfeeding?

Consult your healthcare provider before use while breastfeeding.

Does more D-chiro-inositol mean better results?

No. Research on the 40:1 ratio specifically found that shifting the balance toward more D-chiro-inositol reduced effectiveness rather than improving it.
