Is folic acid the same as folate?
Not quite. Folic acid is a synthetic form the body converts into usable folate, while methylated folate, also called 5-MTHF, is already in the form your body uses directly.† At higher intake levels, research shows 5-MTHF is more bioavailable than folic acid, though current CDC guidance is clear that folic acid remains effective at preventing neural tube defects, a detail worth understanding accurately rather than through wellness-space shorthand.
What folic acid and folate actually are
Folate is the general name for this B-vitamin family. Folic acid is the synthetic, oxidized form typically used in fortified foods and standard supplements, and it requires several conversion steps inside the body before it becomes the active form your cells actually use. Methylated folate (5-MTHF) skips that conversion process, since it is already in the bioactive form.
A 2022 scientific opinion evaluating the bioavailability of 5-MTHF found that at intake levels of 400 micrograms per day or higher, 5-MTHF was more bioavailable than folic acid, with researchers proposing a higher conversion factor for 5-MTHF at these higher intake levels compared with folic acid.
What the MTHFR gene actually changes, according to the CDC
"If we teach women about their bodies and teach them what is actually normal versus what is abnormal, that education can make a real difference earlier on."
— Dr. Samantha Ess, ND, Naturopathic Doctor specializing in hormone health and fertility
The MTHFR gene provides instructions for an enzyme that helps process folate, and variants in this gene are common. Wellness content often claims that people with an MTHFR variant should avoid folic acid entirely and rely on 5-MTHF instead. According to the CDC's current guidance on MTHFR and folic acid, this specific claim is not accurate. People with an MTHFR gene variant can process all types of folate, including folic acid, and folic acid remains the only form shown to help prevent neural tube defects. The CDC further notes that folic acid intake is a more important determinant of blood folate levels than MTHFR genotype itself.
This does not mean methylated folate has no advantage. The bioavailability research above still supports 5-MTHF as an efficiently used form, especially at higher intakes. It means the specific claim that MTHFR carriers must switch to 5-MTHF and avoid folic acid is an oversimplification that current CDC guidance does not support, and it is worth knowing the difference between "5-MTHF is a bioavailable, well-utilized form" and "folic acid is unsafe for MTHFR carriers," which are two different claims.
"Every Pink Stork product is not only backed by science, it's also covered in prayer. That means getting the details right matters to us, even the ones that don't fit neatly into a marketing headline."

Where methylated folate fits in a prenatal routine
Total Prenatal, formulated for first-trimester sensitivity, uses methylated folate (5-MTHF) rather than folic acid, supporting healthy fetal neural tube development in a form the body uses directly.† Pink Stork Premium Fertility Gummies, formulated for preconception nutrition, also delivers methylated folate as part of 13 fertility-focused nutrients for women trying to conceive.† Both formulas reflect a preference for the bioavailable form of folate, grounded in the bioavailability research above, without overstating what MTHFR genotype specifically changes.

Frequently asked questions
Is folic acid the same as folate?
Not exactly. Folate is the general term for this B-vitamin family, while folic acid is a synthetic form that requires conversion in the body, and methylated folate (5-MTHF) is already in the bioactive form.
Is methylated folate more bioavailable than folic acid?
Research evaluating intake levels of 400 micrograms per day or higher found 5-MTHF was more bioavailable than folic acid at those levels.
Should I still choose a prenatal with methylated folate?
Many women prefer methylated folate for its bioavailability profile, though this is a preference based on absorption research rather than a requirement tied specifically to MTHFR genotype.
Is Total Prenatal safe to take during the first trimester?
Total Prenatal is designed for preconception through breastfeeding, including the first trimester. Always consult your healthcare provider before starting any new supplement, especially during pregnancy.†
Are Premium Fertility Gummies safe during pregnancy?
No. Premium Fertility Gummies are not intended for use during pregnancy. Always consult your healthcare provider before starting any new supplement.†
