Why does methylated folate matter in a prenatal vitamin?

Methylated folate, also called 5-MTHF or methylfolate, is the form of folate your body can use directly, without extra conversion steps.† That matters in a prenatal because folate supports healthy fetal neural tube development.† Folic acid, the synthetic form, must be converted before the body can use it, and how efficiently that happens differs from person to person.

What is methylated folate?

Folate is a B vitamin your body needs for healthy cell division, which is why it carries so much weight before and during pregnancy.† Folate comes in several forms. Food folate occurs naturally in leafy greens and legumes. Folic acid is the synthetic form found in many fortified foods and supplements. Methylated folate (L-5-methyltetrahydrofolate) is the active form that circulates in the blood.

For a closer look at how the two supplement forms compare, read our comparison of folic acid and methylfolate during pregnancy.

Pink Stork formulates our prenatal with methylated folate and gentle iron for this reason. It uses 5-MTHF so the folate in each serving is already in its usable form.†

Always consult your healthcare provider before starting any new supplement, especially during pregnancy, breastfeeding, or while managing a medical condition.

How is methylfolate different from folic acid?

Your body converts folic acid into methylfolate through a series of enzyme-driven steps. A published review in the journal Xenobiotica describes how folate forms differ in the way they are absorbed and metabolized. The review is a good starting point if you want the biochemistry behind the comparison.

Methylfolate skips those conversion steps. Because it arrives in the active form, it is a practical option if you want a prenatal that does not depend on your own conversion rate.† Neither form is a substitute for a conversation with your provider about which is right for you.

Why does folate matter so early in pregnancy?

The neural tube forms in the first weeks after conception, often before a pregnancy is confirmed. This is why the Centers for Disease Control and Prevention recommends that all women who can become pregnant get 400 micrograms of folic acid every day. The NIH Office of Dietary Supplements offers a plain-language overview of folate needs, food sources, and supplement forms.

Folate also plays a role in the production of red blood cells and in healthy DNA synthesis, which is why a prenatal covers it at every stage of pregnancy.†

"When I formulate for a mom, I ask what I would want for my own family. Science guides what goes in the bottle, and prayer covers every woman who opens it."

What does an MTHFR gene variation have to do with it?

MTHFR is the gene that provides instructions for an enzyme involved in processing folate. MedlinePlus Genetics explains that common variations in this gene are well documented. If you have questions about your own MTHFR status, ask your provider whether testing makes sense for you. Do not assume a result from an at-home report tells the full story.

Methylated folate is the bioavailable form your body uses directly, which is one reason many women choose it.†

What should you look for in a prenatal with methylated folate?

Start with the label. Check that folate is listed as L-5-methyltetrahydrofolate or 5-MTHF, then look at the rest of the formula.

  • Iron form: Iron bisglycinate chelate supports healthy iron status during pregnancy and is formulated to be gentler on digestion than ferrous sulfate.†
  • Choline: Choline supports healthy fetal brain and spinal cord development.†
  • DHA: Algae-sourced DHA supports healthy fetal brain and eye development.†
  • Testing: Look for third-party testing and cGMP-certified manufacturing.

Total Prenatal, a 22-nutrient blend with ScentCert technology, includes all of these. ScentCert is designed to reduce scent-triggered nausea during pregnancy. If nausea is already a concern, our guide on whether ginger helps with morning sickness covers the research.

Total Prenatal is third-party tested at ISO 17025 accredited labs, made in cGMP-certified facilities, and available at Target, Walmart, and CVS. It also has the support of more than 50,000 verified Amazon reviews across the Pink Stork brand.

When should you start taking folate before pregnancy?

Because the neural tube forms so early, most guidance points to starting folate before you conceive. Our guide on how long before pregnancy you should start folate covers timing in detail.

If you are still trying to conceive, our fertility gummies with methylated folate and myo-inositol offer methylated folate in a gummy format for preconception nutrition.† Premium Fertility Gummies are not for use during pregnancy. Ask your provider when to move to a prenatal.

How do you choose between prenatals with methylated folate?

Compare labels on four points: the folate form, the iron form, whether DHA is included, and whether the brand shares third-party testing. Then ask your provider or pharmacist to review your full supplement list, since your needs depend on your health history.

If you want a single formula designed for preconception through breastfeeding, consider our prenatal with methylated folate, choline, non-constipating iron, and algae DHA.†

Frequently asked questions

Is methylfolate the same as folate?

Methylfolate is one form of folate. It is the active form your body uses directly, while folic acid is the synthetic form that must be converted first.†

Is 5-MTHF the same as methylated folate?

Yes. 5-MTHF, L-5-methyltetrahydrofolate, and methylfolate all refer to the same active form of folate.

Do I need methylated folate if I do not have an MTHFR variation?

You do not need to know your MTHFR status to choose methylated folate. Ask your provider which form fits your needs.

Can I get enough folate from food alone?

Leafy greens, legumes, and fortified foods all contribute folate. Many providers still recommend a prenatal supplement, so ask yours what fits your situation.

Does Total Prenatal contain folic acid?

Total Prenatal uses methylated folate (5-MTHF) as its folate source.†

Can I take a prenatal before I am pregnant?

Many women start a prenatal in the months before conception. Total Prenatal is designed for preconception through breastfeeding.† Check with your healthcare provider about timing.
